Survey: Do you use a service animal?
Are you a US resident (Age 18+) who uses a service animal or other animal that assists you with your disability? If not, are you the parent of a child who uses such an animal?
If so, the Southwest ADA Center (www.southwestada.org) would like to learn about your experiences with accessing public places with your animal. We invite you to complete the following anonymous survey at:
http://www.surveygizmo.com/s3/3773597/Assistance-Animal-Survey
The information gathered from the survey will help the Southwest ADA Center
- understand what access issues people are having with their animals; and
- to develop information, support, and guidance needed for the community to improve access for users of assistance animals.
These findings will also improve the trainings and technical assistance that the Southwest ADA Center can provide to the public on this topic.

Community Survey 2017-18
What matters to you? Take our survey and let us know!
Every four years, Disability Rights Texas asks people with disabilities, their families and friends, disability advocates, and others to tell us about things that are most important to people with disabilities in their everyday lives. You can help by taking a few minutes to complete this brief survey.
If you are helping someone with a disability complete this survey, please let the person choose what TWO (2) STATEMENTS are most important to them. Also, when you complete the “About You” section at the end, please answer those questions based on the person you are helping.
If you are a parent, guardian, advocate, or professional, you are invited to complete your own survey based on your experience and relationships with the people with disabilities in your life.
Our brief survey is available online in English, Spanish and Vietnamese.
